Billing Services

As a courtesy to our clients, Genotox Laboratories invoices clients, patients, Medicare, Medicaid, and third-party carriers.

PATIENT BILLING
Patients can be billed directly for laboratory tests performed by Genotox. Complete all required information on the requisition form available HERE, including placing an "X" in the "Self-Pay" area and providing the patient's complete address and phone number.

THIRD-PARTY BILLING
Genotox bills insurance companies directly if the appropriate information is provided with the specimen. The patient is responsible for payment regardless of the filing of any insurance claim. Please call Genotox’s billing team at 1-844-797-9647 if you have any questions. Genotox does not contract with HMOs.
